Registered Nurse- Patient Aligned Care Team (PACT)
The Patient Aligned Care Team (PACT) Registered Nurse (RN) duties include but are not limited to:
Serves as the lead of the integrated care team established by Primary and Specialty Medicine. Provides clinical leadership and expertise on Primary and Specialty Medicine processes and regulations. Serves as a liaison to both internal and external providers and will be responsible for managing the care of the Veteran and the seamless transition to and from the community. Follows the standardized care coordination model, incorporating case management principles if applicable, knowledge of processes related to referral and authorization and secure medical documentation exchange. Identifies Veterans requiring disease management and the process to obtain these resources internal or external to the VA if applicable. Provides care that is unique to the Veterans needs and continues to monitor, track and intervene if necessary in the episode of care across the continuum. Demonstrates effective communication skills to collaborate and co manage the Veterans care with the multidisciplinary team in a cost-effective manner. Provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process that includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well-being; and executing medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider. The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Hearing (aid permitted). Ability to work around moving objects or vehicles. Ability to work closely with others. Protracted or irregular hours of work. Ability to handle hazardous drugs.
