The APRN functional role includes following the standardized care coordination model, incorporating case management principles if applicable, knowledge of processes related to referral, authorization, and secure medical documentation exchange.
Other duties included but are not limited to:
Support the activities of Primary and Specialty Medicine (PSM).
Responsible for planning, integrating, implementing, modifying, and evaluating the effectiveness of Veteran's care.
Support delivery of high quality, timely care to Veterans receiving services.
Lead of the integrated care team established by the PSM.
Responsible for providing clinical leadership and expertise on PSM processes and regulations.
Serves as a liaison to both internal and external providers and responsible for managing the care of Veterans and the seamless transition to and from the community.
Possesses the knowledge related to VHA guidelines, utilization review criteria, current evidence-based standards of care, compliance guidelines and local policy related to Delegation of Authority as established by the facility Chief of Staff.

Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment.
You will receive credit for all qualifying experience, including volunteer experience.
The Department of Veterans Affairs performs pre-employment reference checks as an assessment method used in the hiring process to verify information provided by a candidate (e.

, on resume or during interview or hiring process); gain additional knowledge regarding a candidate's abilities; and assist a hiring manager with making a final selection for a position.

Veterans' Preference Note:
Preference eligible candidates and other Veterans will be given preference when qualification of candidates are approximately equal for Title 38 positions in VHA.
