Community Health Worker | Family Health Center | Northway Clinic
Full-time 80 hours every 2 weeks Mon-Fri 8a-430p Bi-lingual English/SpanishJOB SUMMARYThe Community Health Worker (CHW) will primarily be working out in the community with specific target populations.
CHW's will work closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes.
In addition, the CHW will develop and maintain relationships with community-based organizations and referral partners.
Supports and implements patient safety and other safety practices as appropriate.
Supports and demonstrates Family Centered Care principles when interacting with patients and their families and with co-workers.
EDUCATION &
Experience:
Associate's degree required, Bachelor's degree in appropriate subject matter preferred.
Experience in community outreach, preferably with the target audience and culture.
Working knowledge of various systems of health care in the community.
LICENSES AND CERTIFICATIONS Community Health Worker certification upon hire preferred or Community Health Worker certification obtained within 1 1/2 years required.
Basic Life Support (BLS) Current American Heart Association (AHA) BLS Healthcare Provider card per established policy required.
QUALIFICATIONS Prefer experience with culture-related learning principles and effective communication methods.
Bilingual/bicultural (Spanish).
Strong written and verbal communication skills.
Demonstrated experience and ability in providing superior customer service to patients.
Understanding of and commitment to appropriate protection of confidential patient information.
A valid driver's license in good standing required.
