Rounding Geriatric Primary Care Physician Overview:
Conduct on-site patient visits in senior living settings, providing primary care services.
Perform comprehensive assessments, including medical history reviews and physical examinations.
Develop personalized care plans tailored to each patient's needs and goals.
Collaborate with interdisciplinary teams, including advanced practice providers, nurses, and therapists, to ensure coordinated care.
Build trusting relationships with patients and their families, fostering open communication and shared decision-making.
Educate patients and families about their health conditions, treatment options, and self-care strategies.
Monitor and manage patient health, aiming to reduce hospitalizations and optimize overall well-being.
Rounding Geriatric Primary Care Physician Profile:
Valid and unrestricted MD license from the appropriate medical licensing board.
Specialization or experience in geriatric care, demonstrating a comprehensive understanding of the unique needs and best practices for the elderly population.
Prior experience in rounding within senior living environments or as a Hospitalist, showcasing familiarity with the specific challenges and dynamics of providing care in these settings.
Excellent communication skills, both verbal and written, to effectively interact with patients, families, and the interdisciplinary care team.
Strong clinical and diagnostic skills, enabling accurate assessments, appropriate treatment planning, and efficient decision-making.
Proficient in developing and implementing personalized care plans, considering patient preferences, medical history, and evidence-based guidelines.
Valid driver's license and current auto insurance, facilitating mobility and accessibility to senior living communities for on-site patient visits.

We take a coordinated and collaborative approach to keeping residents healthier longer and safer in their communities and avoid unnecessary ER and hospital visits.
Our goal is to alleviate some of the burden and stress that senior communities and families feel when trying to coordinate needed medical services for residents and loved ones.
